Abdominal pain

A 19-year-old woman presents with pain in her lower abdomen, back pain, painful urination, and painful sexual intercourse.  She also complains of nausea and vomiting and that the symptoms got worse during her recent period.   Physical examination shows a mucopurulent vaginal discharge, cervical motion tenderness and a temperature of 37.9.  Below is an image from a similar case.
